Base note, middle note, high note etc etc

Ok so Im new like most others who post here, I have 2 questions:

1) What does the base note, middle note, high note etc mean? Is it the different smells you get from the perfume as it wears on you for longer?

and....

2) I love fragrances that smell like chocolate, vanilla or similar etc what can you recommend? I'll give you a few Ive tried before so that you dont recommend these: Jean Paul Gaultier - Gaultier 2, Givenchy - Very Irresistible, Givenchy - Pi, Dior - Homme, Thierry Mugler - Angel (girls I know but I love it) I dont mind any unisex suggestions or female scents if they could be worn by a guy without too many odd looks. Thanks guys!!!!!!
